Documents required to enrol at Tuncurry Public School

As parents and care providers you will need to bring these documents to the appointment with the school:

For children already enrolled in a NSW public school we require:

• 100-point residential address check list (originals only) - see below
• family law or other relevant court orders (if applicable)
• action plan if your child has anaphylaxis, asthma or other serious allergy (must be original, completed and signed by your GP)

All documents must be supplied before enrolling at Tuncurry Public School.

If your child has  not been previously enrolled in a NSW public school, you will need to bring these documents with you:

• your child's original birth certificate or identity documents (passport)
• proof of your child's address - original (100-point residential address check list - see below)
• immunisation history statement based on the Australian Childhood Immunisation Register (ACIR) (original can be obtained from Medicare)
• family law or other relevant court orders (if applicable)
• action plan if your child has anaphylaxis, asthma or other serious allergy (must be original, completed and signed by your GP)

If your child is not a permanent resident you will also need to provide:

• passport or travel documents
• current visa and previous visas (if applicable).

If your child has any special circumstances, allergies, health or medical conditions, please contact the school and arrange a meeting with the Principal before your child starts school.
